Many of the forms you may need to access are now available online. You can fill in your details online, click submit and it will be sent to the relevant team for processing. 

Some of the forms we have available include: 

• Repairs and Maintenance – report your repairs and attach photos of the issue.   
• Modifications request – outline your access needs and attach your Occupational Therapist report.   
• Alteration request – request permission to carry out an alteration or addition to your property.  
• Pet Application – if you would like to request permission to have a pet at your property.   
• Additional Occupant – if you would like to request adding a member to your household who was not there when you began your tenancy.  
• Absence from Property – if you would like to request approval if you are planning to be away from your property for more than 6 weeks.  
• Ending a Tenancy –If you would like to request ending your tenancy with SGCH,  
• Appeals and Complaints – If you are unhappy with our service or dissatisfied with a decision, you have the right to lodge a complaint or appeal that decision.  

The forms can be completed on a computer, phone or tablet. You can access all webforms here on our site using the links above. All forms are still available in a paper version, too – it’s your choice!
